2. Fill out the form completley. Required fields are in bold.
3. Be sure to include the Student's name, date(s) of absence, reason for absence and all parent / guardian information.
4. Please ask your physician's office to directly e-mail your child's excuse to the email address at the bottom of the form.
5. Please review all information provided for accuracy before submitting this form.
